At your first visit we ask you to complete a few forms for us.

You can complete these forms online, print them out and bring them into your appointment (due to confidentiality issues at this stage we are unable to have you email these forms)

At RTH

If you prefer the forms are available at RTH for you to fill out on the day of your first appointment.
